The primary role of Aged Care Assessment Services or Teams (ACAS/Ts) are to independently and comprehensively assess the care and support needs of frail older people, and to facilitate access to available care services appropriate to their long term care needs.

These services will assess people for access to packages of care to support older people to remain at home.

The Aids and Equipment Program (A&EP) provides people with a permanent or long-term disability with subsidised aids, equipment, vehicle and home modifications to enhance independence in their home, facilitate community participation and support families and carers in their role.
